Advanced Care at Home ensures high-quality care while accelerating children’s safe discharge by offering virtual support at home. The program is intended for children who are stable but require additional medical care, and it is especially helpful for children with complex disorders and other medical problems.
By providing virtual support at home, Advanced Care at Home expedites children’s safe discharge while guaranteeing high-quality care. The program is particularly beneficial for kids with complex diseases and other medical diagnoses, and it is designed for kids who are stable but need more medical attention.
“Advanced Care at Home is an extension of our Whole Child Health approach—care designed to support children where they live, learn, and grow,” stated R. Lawrence Moss, MD, FACS, FAAP, President and CEO of Nemours Children’s Health. “This is an entirely new model of care that brings children home, keeps families together, and lets the entire family be comfortable in their space.”
Nemours Children’s Advanced Care at Home has seen the enrollment of over 120 patients since its launch in June. In addition to helping children recuperate comfortably at home, the initiative has saved families money and stress by returning 177 hospital days, preventing 27 readmissions to the hospital, and averting 91 ED visits.
“We know patients and families benefit significantly from treatment options that enable safe, continuous care and recovery in the comfort of their homes,” stated Jane Mericle, Executive Vice President, Chief Nursing Executive, and Patient Operations Officer of Nemours Children’s Health. “Advanced Care at Home offers tremendous promise in revolutionizing pediatric health care.”
By the end of 2025, the initiative will have spread to Nemours Children’s Hospital in Delaware, and in 2026, a pediatric-focused mobile integrated health program is planned.
